UCCS OIT Project Intake Form

Please complete the following Project Intake Form for consideration by UCCS OIT Leadership Team.

This form is intended to capture project details at a high level. If you have multiple projects, please submit a form for each project you have. Once this form has been completed, please allow up 10 business days for UCCS OIT to review internally. Once your submission(s) have been reviewed, a member of the UCCS OIT Leadership team will reach out to get more information and/or schedule a Project Kick-off Meeting.
